Tailored Respite Services

We Plan It. You Provide It. Respite Reinvented.

At Swan Community Connect, we take the stress out of respite planning so you can focus on what matters most — delivering exceptional care. We’re not an NDIS provider, meaning you keep full control of your participant relationships while we work behind the scenes to support you. Our fee-for-service model offers tailored, participant-focused respite or short-term accommodation (STA) planning that saves you time, reduces admin, and enhances client outcomes. We handle the logistics, coordination, and presentation, allowing you to focus on delivering quality care. It’s respite, reinvented.

We Plan It.

As part of our tailored respite services we deliver:

  • Personalised respite plans focused on participant goals, preferences, and support needs.

  • Suitable, participant-friendly accommodation & capacity building activity bookings.

  • Meal planning & dietary support.

  • Visual schedules & budgeting tools.

  • Ongoing support throughout the planning and STA period.

All our respite and STA programs are:

  • NDIS-goal focused and align with individual participant goals.

  • Clear and participant-friendly.

  • Smooth and stress-free .

You Provide It.

Once the plan is ready to go:

  • Use your own trusted support workers.

  • Your support workers use the tailored program we’ve created to guide the delivery of meals, activities, and support during respite.

  • We are on hand to support any quick adjustments to bookings or schedules.

  • Provide post-STA feedback for continuous improvement.

  • We stay in the background; you get the credit.

Respite Reinvented.

By choosing Swan Community Connect, you’re not just getting a tailored, hassle-free respite program, you’re also giving back to the community.

As a not-for-profit organisation, 100% of our proceeds are reinvested locally, funding:

  • Free wellness and wellbeing workshops.

  • Domestic and family violence advocacy.

  • Inclusion and community-connection projects.

  • Mental health and peer-support programs.

  • Youth and family development initiatives.

  • Accessible, community-led events.
